  1. Yes . . . he is a twin, and I imagine twins tend to be smaller in the US, as well. But children are often born with low birth weights. (I know very few who are more than 7 pounds at birth.) Malnourished moms tend to give birth to malnourished children. Childhood malnutrition, particularly from conception to age 2, contributes to life-long health and cognitive difficulties.
